A PCMH is a care community that collectively invests in and takes accountability for your child’s care with a holistic mind and body approach. In this personal model of health care, your provider and the extended team of health care professionals build a relationship with you, learn about your family and individual circumstances that may affect health outcomes, and understand your medical history and concerns, creating a trusting relationship, establishing trust and providing a dependable resource for
evidence-based health care tailored to your families’ needs.
Pediatrica providers are licensed and board certified in Pediatrics; we are guided by evidence-based care and use the most up to date technology to deliver care. Your child’s provider will ensure to every aspect of your child’s health through assessment and discussion, and they will work with you to develop a care plan to ensure the best outcomes for your child. The Pediatrica team is committed to fostering a relationship between providers, care teams, and patients that will promote better overall healthcare.
Your child’s care team is composed of clinical and non-clinical team members who facilitate the development and coordination of individual care plans. If your child requires evaluation by a qualified provider or specialist outside of our practice, including behavioral health, the Pediatrica care team will assist in obtaining appointments, referrals, and will submit the appropriate records to the specialist for further evaluation and treatment. In the event that your child is evaluated and treated in the emergency room, your care team will make sure that proper follow-up care is coordinated with us as your medical home, as well as any necessary specialists. In order to promote the highest level of continuity of care, we ask that you inform us of any changes to your child’s medical history or of any evaluations and treatments outside of our practice.
We invite you to schedule an appointment for physicals, sick visits, and immunizations by calling our office during business hours or through the secure patient portal. For same day appointments, please call our office directly. Should you need to cancel or reschedule your appointment, please provide us with as much notice as possible as a courtesy to all families at our practice.
If you require clinical advice during business hours, please call our office. Upon receipt of your call, a provider will respond to urgent messages within 3 hours and to non-urgent messages by the next business day.
Access to clinical advice during and after hours is a fundamental principle of the medical home. If your child should have an emergency after hours, during weekends, or on holidays in which we are closed, we provide an answering service. An answering service that has pediatric-trained nurses answering the calls with access to your chart. These nurses can give recommendations for at-home care, advice when an urgent care or facility is necessary, and can even help set up an appointment. All information given through the answering service is relayed to our medical staff and follow up
appointments can be scheduled when necessary.
For your convenience, we offer a secure patient portal through which you can send us secure messages, access your child’s health record, and request medication refills. During your visit with us, staff will take care of registering you for the portal and demonstrating how to navigate through it.
