Complete care for our patient famlies means caring about our communities, too. Through Pediatrica Foundation Works, we are establishing programs and resources that extend beyond our exam rooms to fuel our communities through connection, access, and partnership.

Hope Rises: Community Sponsorship Program

At Pediatrica Health Group, we believe strong, healthy communities start with supporting the kids and families who live in them.
That’s why we created Hope Rises – a community sponsorship program designed to lift up local groups making a difference for youth.

Each quarter, every Pediatrica practice contributes up to $500 in funding to help grassroots and not-for-profit organizations that foster youth wellness, accessibility, inclusion, and community connection through activities like sports, recreation, education, and health initiatives.

Funding Amount: Up to $500 per practice per quarter.

Who Can Apply: Local not-for-profits, community groups, or community ambassadors (e.g., youth coaches, program coordinators) serving children, youth, or families.

Application Cycles:

  • Q1 (Jan–Mar) → Apply by Dec 31
  • Q2 (Apr–Jun) → Apply by Mar 31
  • Q3 (Jul–Sep) → Apply by Jun 30
  • Q4 (Oct–Dec) → Apply by Sep 30

Eligibility Criteria

To qualify, applicants must:

  • Be a registered not-for-profit or an unincorporated community group operating within 15 miles of a Pediatrica practice
  • Aim to enhance youth wellness and development through community engagement offerings and/or program facilitation. Funding may not be used to fund promotions, propaganda, or coerce participation or membership in any particular group
  • Serve children, youth, or families as their primary population
  • Offer an accessible or inclusive component, such as:
    • Free or low-cost programming
    • Sliding-scale or financial assistance options
    • Inclusion of youth with disabilities, newcomers, or underserved populations

Impact

Recipients agree to share a brief impact story after the funded activity and to participate in social media activities and other recognition, which may include a check presentation opportunity or a community highlight feature on our website or social channels.
